Your privacy matters to us, and this policy explains in plain language what information this website collects, why we collect it, who we share it with, and the choices you have. It applies to aervita.com and to information you submit through it. It does not replace the notices and consents you receive as a patient of our clinic, which are provided separately when you become a patient.
Information you give us. When you fill out a contact form, request our free guide, book an evaluation, join our email list, or place a deposit, you provide information such as your name, email address, phone number, and the contents of your message. If you choose to tell us about your health interests or goals in a form or message, we receive that too, and we treat it with the heightened care described below.
Payment information. When you place a deposit or make a purchase through this website, your payment is processed by our practice management and payment processing providers. We do not store your full card number on our own systems.
Information collected automatically. Like most websites, we use cookies and analytics tools, including Google Analytics, that collect information such as the pages you visit, how you arrived at our site, your device and browser type, and general location at the city level. This information helps us understand what content is useful and improve the site.
We use your information to respond to your inquiries, schedule and manage appointments, deliver the resources you request such as our educational guide, process payments and deposits you initiate, send you emails you have signed up for, operate and improve the website, and comply with our legal obligations. We do not sell your personal information, and we do not share it with third parties for their own advertising.
We are a healthcare clinic, and we treat any health related information you share through this website with heightened care. Information you submit through this site is used only to respond to you and to prepare for your care. Once you become a patient, your health records are maintained in our clinical systems and protected in accordance with applicable law, including the Minnesota Health Records Act, and are governed by the patient privacy practices we provide to you directly. Please do not submit detailed medical information through general website forms. The evaluation visit is the right place for that conversation.
We share information only with the service providers that make this website and our clinic work, and only for that purpose. These include our practice management and booking platform, our payment processor, our email delivery service, and our website analytics provider. Each is permitted to use your information only to provide services to us. We may also disclose information when required by law, to protect the rights and safety of our patients and staff, or in connection with a business transaction such as a sale or reorganization of the practice, in which case this policy would continue to apply to previously collected information.
If you sign up for our email list or download our guide, we will send you educational content and clinic updates. Every marketing email includes an unsubscribe link, and unsubscribing stops marketing messages without affecting communications about appointments you have scheduled. If you provide your mobile number, we may communicate with you by text about your inquiries and appointments. We will not send you marketing text messages unless you have separately agreed to receive them, and you can stop text messages at any time by replying STOP.
You can control cookies through your browser settings, and the site will continue to work with cookies limited, though some features may be less convenient. Google provides an opt out for Google Analytics at tools.google.com/dlpage/gaoptout.
Depending on where you live, including under Minnesota’s consumer data privacy law, you may have the right to know what personal information we hold about you, to receive a copy of it, to correct it, to delete it, and to opt out of certain uses such as targeted advertising, which we do not engage in. To exercise any of these rights, contact us using the information below and we will respond within the time required by law. We will never treat you differently for exercising a privacy right. Requests concerning your medical records as a patient are handled under the health records process described in our patient notices, which provides you additional rights under Minnesota law.
This website is intended for adults and is not directed to children under 13. We do not knowingly collect personal information from children through this website. Care for minors at our clinic is arranged by a parent or guardian directly with our office.
If we change this policy, we will update the effective date above and post the revised version here. Material changes will be noted prominently on this page.
